The HAP and April Subsidy Programs provide rental assistance and housing support to 66 individuals and families at any given time. HAP also provides housing start up loans to approximately 60 families a year. The housing support workers providing these services go out of their way to work with landlords, secure housing, and explain tenants’ rights and responsibilities for persons on these programs. Housing support workers routinely do housing checks on the tenants to ensure all is well and to make any referrals as needed. |

The HESIP Program (Housing and Employment Services Integration Program) is a collaboration between Bridges (empowerment agency for the disabled); NEIGHBORING Mental Health Services (vocational services); Pathways, Inc. (mental health services); and the Recovery Program (through the Lake County ADAMHS Board). This extremely flexible program’s purpose is to move from silos of housing, employment and supports to integrated strategies to support recovery, community interdependence, and collaboration among service providers. People with severe and persistent mental illness will have access to permanent housing of their choice via the acquisition of paid competitive employment. Monies from this program are used to meet the following consumer needs: education, transportation, employment start up costs (uniforms, clothing, etc.), rental assistance, housing start up costs, emergency rent (security deposits, short term rent up to 3 months) and rental subsidies (75 households total). Through collaboration and the HESIP Participant’s willingness to work on employment goals, this program’s success rate is encouraging !

The purpose of the Homeless Outreach Program is to link a newly identified homeless person or persons facing homelessness with mental health and housing services. Through intensive outreach The Homeless Outreach Program provides necessary supplies such as hygiene products, food vouchers and bus tokens. Once a person becomes enrolled in the program the outreach worker provides support with but not limited to shelter, mental health services and entitlements. |

The Shelter Plus Care Program, funded through HUD, provides 25 rental housing vouchers for homeless persons with a severe and persistent mental illness. Participants must receive mental health and recovery services as a condition to receiving a Shelter Plus Care housing voucher. Funding for this project was obtained through the collaborative efforts of the Coalition on Housing and Support services of Lake County, the Lake County ADAMHS Board, and the United Way of Lake County. |

Properties: Extended Housing, Inc. owns and manages 18 properties throughout Lake County. These properties are purchased with capital dollars from HUD, the Ohio Department of Mental Health, the Ohio Department of Development, the Lake County Commissioners and the Lake County ADAMHS Board. Extended Housing, Inc.’s properties include three transitional group homes, apartment houses/complex, condos, and single family homes, totaling 94 units. |
